    The Competence Centre for Epidemiology and Health Services Research for Healthcare Professionals (CVcare) was established on 1. October 2010 with the appointment of Prof. Dr. Nienhaus to an endowed professorship founded by the Statutoy Accident Insurance in the Health and Welfare Services (BGW). By funding the endowed chair the BGW aims to promote separate and independent
    research in the field of workrelated health promotion and prevention and rehabilitation for employees in the health and welfare services in accordance with its statutory mandate.

    The CVcare provides epidemiological data on the working conditions and health of nursing staff and other employees in the health and welfare sector. Work-related health promotion, prevention and rehabilitative measures for these occupations are evaluated in terms of health services research. Both the requirement for new measures and the effectiveness of existing ones are investigated. The key base data is derived from information routinely collected by the social insurance schemes (accident, health and pension insurance), registered findings of preventive medical check-ups by company doctors in accordance with the German Occupational Medical Surveillance Directive (ArbMedVV) as well as surveys and controlled studies carried out by the department itself. Offers of occupational health promotion, prevention and rehabilitation for employees in the health sector are critically scrutinized.

Special Issue: The Burden of COVID-19 in Workers

Invitation from the International Journal of Environmental Research and Public Health (IJERPH) to submit papers

SARS-CoV-2 case rates continue to rise worldwide, increasing the risk of occupational infections in the workplace. Healthcare workers are at the highest risk of infection due to their close contact with COVID-19 patients. This raises the question of how to separate a work-related risk from a private or background risk in a pandemic. COVID-19 is a multi-organ disease with long-lasting negative effects on the health of those who become infected, including workers. Therefore, it is of utmost importance to assess the impact of COVID-19 in the workplace on health, quality of life and ability to work.

Coordinated by Prof. Albert Nienhaus (CVcare/BGW), the International Journal of Environmental Research and Public Health (IJERPH) will dedicate a special edition to this topic. In the literature, the assessment of work ability and compensation of workers after COVID-19 infection has hardly been discussed so far. For this special issue, researchers and experts will be encouraged to contribute to the questions of which workers are most commonly affected and how to assess the health status of workers after COVID-19 infection so that fair compensation can be awarded for the long-term effects on health, quality of life and ability to work.

Occupational health and workers' compensation experts from different countries with different social security systems are invited to submit their contributions by 30 December 2022.

    Employees in the health care system are subject to an increased risk of infection. Numerous, sometimes very different pathogens play an important role in this. Known infectious diseases such as tuberculosis (TB) or hepatitis continue to play a major role in occupational diseases, although reports of infections with multi-resistant pathogens (MRE), especially methicillin-resistant Staphylococcus aureus (MRSA), are increasing. The risk posed by the infectious agents usually occurs unexpectedly or imperceptibly, which makes it difficult to assess the causal link. In addition, data on the epidemiological situation are often lacking. The aim of CVcare's infection studies is to close these gaps in knowledge. In addition to questions about the infection risks of individual occupational groups or areas of work such as Helicobacter pylori in gastroenterology employees, the data of the TB company physician network were systematically collected and evaluated. A further focus is on the occupational risk of hepatitis C among medical personnel and the investigation of the quality of life of employees with a professionally acquired hepatitis infection. In the MRSA research focus, employees in inpatient nursing care for the elderly, outpatient nursing services as well as employees in patient transport and in patient transport are examined for MRSA colonisation and interviewed about risk factors.

    Musculoskeletal disorders (MSDs) are among the most common ailments in the German population and cause high direct and indirect economic costs due to expenditure on treatment, sick pay and (early) retirement as well as absenteeism, loss of production and reduced competitiveness. Last but not least, this results in high costs for those affected, such as physical functional limitations, chronic pain and loss of quality of life. Epidemiological studies and health reports show that a not inconsiderable proportion of MSE can be caused by occupation-specific physical and physical, but also by psychomental effects. The spectrum of symptoms to be investigated and the corresponding pathogeneses are just as diverse as the methodological approaches to analysing these conditions and their causes. Furthermore, there is a need for further research with regard to special occupational groups within the health and social sector in Germany. CVcare investigates various questions on the prevention and rehabilitation of MSE with the intention of drawing attention to existing problem situations, providing practical recommendations and instruments, clarifying and supporting evidence-based decision-making and evaluation processes.

    In recent years, psychological stress and strain have increased, for example due to high work intensity and pressure to take responsibility in the health care professions. CVcare has set itself the goal of evaluating the "hidden" psychosocial stress factors among employees in the health and welfare professions. The aim is to create concrete starting points for preventive measures to avoid psychosocial stress and to clarify the need for new offers.

Current CVcare Projects

"ZeigRückgrat" (Show backbone) is the name of a study undertaken by the CVcare and the statutory accident insurance Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege (BGW) to better understand the widespread disease "back pain" for the benefit of all patients in the future.

The study is aimed exclusively at women between the ages of 40 and 65. Structural changes in the spine of women in physically stressful and less stressful occupations are examined with the aid of magnetic resonance imaging (MRI). Our aim is to better understand occupational diseases caused by stress, so that the assessment of the disc-related disease of the lumbar spine can be facilitated.

    ErgoHair is a cooperation project promoted by the European Commission between Coiffure UBK/UCB, a national employers' organisation from Belgium, Coiffure EU and UNI global union hair & beauty, two umbrella organisations of employers and employees at European level, Les Institutions de la Coiffure, an insurance company for hairdressers in France and CVcare. The aim of this project is to promote healthy and secure jobs in the hairdressing sector. The aim is to promote a joint exchange between European sectoral social dialogue committees in order to contribute to the harmonisation of occupational health and safety, with particular emphasis on ergonomic workplace design and equipment, and the promotion of effective ergonomic working practices in small and medium-sized enterprises. The overriding objective is to raise awareness of the stresses and strains of hairdressers' work through the development and dissemination of preventive ergonomic recommendations and standards for specific target groups and tasks and, as a consequence, to reduce the number of occupational musculoskeletal disorders (MSDs) and complaints (MSBs) in the hairdressing sector throughout Europe. To this end, three workshops are to be held over a period of 24 months. The first and second workshops took place in October 2017 in Hamburg and in April 2018 in Paris. The third workshop will take place in Brussels in April 2018.

    Infection risk of latent tuberculosis in medical personnel after assignment abroad

    The risk of contracting Mycobacterium Tuberculosis in the sense of latent tuberculosis during a foreign mission has not yet been sufficiently investigated. In order to gain new insights into the infection risk of latent TB after an assignment abroad, CVcare is conducting a study in cooperation with the BGW.

    Little is known about the prevalence of a latent tuberculosis infection among medical personnel working in direct patient care in development aid. We assume that the risk of contracting Mycobacterium Tuberculosis in this group is significantly higher than for medical personnel in Germany. Within the scope of the study, the following questions will be answered:

    • How common is a latent tuberculosis infection among medical personnel who have worked in a highly prevalent area under difficult working conditions?
    • How much higher is the risk of a latent tuberculosis infection among medical personnel after an assignment abroad compared to employees in medical institutions in Germany?
    • How high is the probability of converting from a negative to a positive IGRA test result after a foreign assignment?

  • The Institute for Prevention and Occupational Medicine of the German Social Accident Insurance (IPA) at the Ruhr-University Bochum is conducting a cross-sectional study under the direction of Prof. Dr. Monika Raulf in cooperation with CVcare on the recording and evaluation of the prevalence of sensitisation and allergic complaints in the respiratory tract and skin among veterinary specialists (TFA). In addition to the health aspects, the current allergen exposure at the workplace is also determined and compared with those in the domestic environment.


    Allergen exposures play an increasingly important role in the world of work. The professions with an increased risk of sensitisation also include veterinarians and veterinary assistants. In addition to the risk of infection and injury when handling animals, exposure to animal, plant and microbial allergen sources as well as chemical substances such as disinfectants occurs in various areas of work. In particular, intensive contact with various farm animals and pets can lead to sensitisation and allergic diseases among employees. Within the framework of this pilot study, veterinary assistants in the greater Ruhr area are to be examined.


    In the participating veterinary practices and in the households of the participants, dust samples are collected with the help of passive collectors and analysed for typical allergens. In addition, the activities in the veterinary practices will be examined for their allergen exposure and the current allergic symptoms of the TFA will be determined by medical and serological findings. The sensitisation of the TFA will be determined by a questionnaire which collects data on the patient's own and family history, allergic diseases in childhood and adolescence, smoking behaviour and duration of activity. The aim is to record and evaluate the frequency of sensitizations against ubiquitous and workplace-relevant allergens among veterinary specialists, as well as the current allergen exposure at the workplace and possible carry-over of occupational-relevant allergens into the private sphere..

  • The topic "violence in care" is mostly understood as violence against elderly and cared for people. But nursing staff are also victims of verbal or physical abuse. The results of an EU-wide survey by the European Agency for Safety and Health at Work (EU-OSHA) (2002) show that health care is one of the sectors most affected by violence at work. In the care sector, verbally aggressive behaviour is much more frequently reported than physical assault. In the study "Neue Wege bis 67 - In der Dienstleistung bis zur Rente" (New ways up to 67 - In the service sector up to retirement age), about half of the nursing staff interviewed stated that they had been exposed to physical aggression by people in need of long-term care in the past twelve months; 75% reported verbal aggression against them. Every fourth person was even confronted with verbal violence on a daily basis. Although the verbal threats, intimidations and insults that frequently occur in everyday care have no direct physical effects, they can be perceived as very stressful by the nursing staff concerned. A study by the Berufsgenossenschaft für Gesundheitsdienst und Wohlfahrtspflege (BGW) has shown that nursing staff with a migration background are more often victims of verbal attacks by senior citizens than local nursing staff. Nursing staff with a migration background are also often exposed to racist insults and insults. This results in an urgent need for a differentiated scientific examination of this topic. In order to gain an overview of the occurrence of verbal assaults among nursing staff with a Turkish migration background, the Competence Centre Epidemiology and Care Research for Nursing Professions (CVcare) at the University Hospital Hamburg-Eppendorf (UKE) is conducting a study. Within the framework of this study, the way in which this is dealt with and the consequences thereof will also be investigated and highlighted. Employees with a Turkish migration background were chosen because they represent one of the largest groups of nursing staff with a migration background.

    For the study, CVcare is looking for nursing staff with a Turkish migration background who are willing to be interviewed. The interview lasts about 30 minutes and all statements are treated confidentially. Neither the management nor the work colleagues get an insight into the interviews.     The aim of this study is to gain new insights into the frequency, type and severity of aggressive attacks in nursing and care professions. The study also examines the extent to which support measures are implemented to de-escalate violent attacks and how they affect the workforce as a whole. The study results serve to describe the extent of the problem and provide important insights into the need for and implementation of preventive measures in nursing and care facilities. The following concrete questions should be answered:

    • How often are care and nursing workers confronted with physical and verbal violence and aggression in the workplace?
    • What are the nature and severity of the assaults, and what impact do they have on workers?
    • What changes have there been in comparison with a 2009 survey on violence and aggression and their impact on workers in the occupational groups surveyed?
    • What support services are available to employees in the facilities for preventing and dealing with violence and aggression?
    • To what extent are these offers taken up and evaluated as preventive and helpful in dealing with violent and aggressive attacks?
    • Are there differences in the type and frequency of assaults against caregivers with and without a migration background?

  • The aim of this project is to compare a control and an intervention group in order to examine whether the integration of behavioural and relational prevention modules on the subject of occupational health and safety into the day-to-day training of nursing staff for the elderly leads to an improvement in their health and safety competence and has an effect on their job satisfaction.

  • The aim of the study is to investigate the exposure of employees to heavy metals at workplaces used for screen recycling in workshops for disabled people (WfbM). For this purpose, the total pollutant load is determined, which can be caused by inhalative, dermal and oral input paths. The inhalation rate is determined by air measurements in the workrooms. The entry from dermal and oral admission is carried out by biomonitoring of the employees through a urine test and is supplemented by answering a questionnaire.

    Every year, large quantities of electronic scrap are produced, which contain a large number of heavy metals that are harmful to health. Workers are exposed to this exposure during recycling. This also applies to the recycling of screens. First-generation flat-panel LCD screens contain mercury-containing components for backlighting. Other heavy metals are contained in cathode ray tubes or printed circuit boards. The screens are manually dismantled during disposal. Some of these activities are carried out by employees in workshops for disabled people (WfbM). Many heavy metals are classified as hazardous substances at the workplace and can have a skin resorptive, reproductive toxic and/or carcinogenic effect. Previous investigations have been limited to the detection of mercury in air and urine at comparable workplaces. The extension to a larger group of heavy metals allows a more comprehensive insight into the workplace exposure of employees during screen disassembly.

    While in the past decades gram-positive pathogens such as methicillin-
    resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococci (VRE) in the
    The increased incidence of multidrug-resistant patients has moved to the forefront of interest,
    gram-negative pathogens (MRGN). Due to their adaptability and development of resistance, these pathogens represent an important problem in infection medicine. The consequences are limited to missing therapy options, longer treatment times, higher costs and higher mortality.


    So far, knowledge about MRGN in the professional context is low. A systematic preparation of the current scientific literature should represent the current state of research on occupational exposure. On the basis of this work it is to be examined whether employees in the health service have an increased occupational risk of infection by MRGN.

  • Systematic Review

    By the end of 2018, a study on the prevention of musculoskeletal disorders and discomfort in dentists will be conducted. This study focuses on current approaches to primary and tertiary prevention of MSE/MSB in dentistry. The aim of this study is to identify primary studies that provide information on suitable ergonomic and rehabilitative measures and services for the prevention of MSE/MSB in dentists. The existing literature on this topic will be systematically recorded, evaluated and analysed. This review will be published in English and German in specialist journals.

  • Due to political and social developments, the number of refugees and homeless people living in Germany has risen significantly in recent years. Employees in social work support, advise and care for these people in various social institutions, such as hostels, day-care centres and outpatient counselling centres. There they are in close contact with the residents and clients who find themselves in particularly precarious situations. As a result, the employees are repeatedly confronted with their fates and traumas and are exposed to high emotional demands. Overall, however, little is known from the literature to date about the working conditions and health effects for the employees. The overarching aim of this study is therefore to comprehensively examine the work and health situation of employees in social work with people in precarious situations. The focus is on the employees in the help for fugitives and homeless people. In a first step, a scoping review provided an overview of the scientific literature on work-related stress and resources, mental health and coping strategies of workers in the field of refugee and homeless assistance. Both national and international literature was included. In order to further investigate the pressures, coping strategies and support needs, 26 qualitative interviews were carried out with employees in social work in the fugitive and homeless aid sector in Hamburg and Berlin. The interviews are based on a semi-structured guideline and are currently being evaluated with the help of the qualitative content analysis according to Mayring (2000). A cross-sectional study will then be carried out on the basis of the results. This is to begin in the form of an online survey in autumn/winter 2018. The aim is to verify the results of the interviews with the help of a broad-based survey using standardised survey instruments and to raise them to a representative level.
    On the basis of the overall analysis results, recommendations for action and health promotion measures for employees in social work will be derived. The study results will also be presented at specialist congresses and published in scientific journals.

  • The activity in palliative care and the care of terminally ill people is an important task. In 2015, a pilot project on the work situation and health of nursing staff in palliative care in Rhineland-Palatinate was carried out by the Institute for Occupational, Social and Environmental Medicine at the University Medical Center Mainz. The findings will now be tested in a nationwide research project in cooperation with the Competence Centre Epidemiology and Care Research for Nursing Professions (CVcare) of the University Hospital Hamburg-Eppendorf.

    The aim of this project is to determine the stresses, strains and resources of nursing staff in palliative care throughout Germany. The survey was conducted in the field of nursing staff with and without special training in palliative care,

    • who are involved in specialised palliative care (hospices, palliative wards, SAPV, outpatient hospice counselling services) or
    • who work in general palliative care (hospitals, inpatient care facilities and outpatient care services).

    The nursing staff are interviewed by means of a written questionnaire. The questionnaires can be answered either on paper or online. The answer takes about 20 minutes.

  • Infections with blood-borne hepatitis B and C viruses are among the most common infectious diseases worldwide. Virus hepatitis is therefore a common occupational disease. For the statutory accident insurance institutions, it is a matter of concern to ensure the care of insured persons with a chronic hepatitis further.

    We are conducting this study to determine how chronic hepatitis infection affects the health-related quality of life and employability of employees and former employees. This study will also examine how the measures of the inpatient
    treatment will affect these factors.

Journal Club

The CVcare Journal Club takes place on the first Tuesday of each month. The Journal Club discusses articles that deal with the topics of health care, care research, occupational medicine, prevention and rehabilitation as well as special methodological approaches. In addition to short presentations, the main focus is on the discussion of strengths and weaknesses of the methodology of individual studies.

Interested employees of the UKE who deal with topics related to the world of work are invited to participate. The participants will be informed about the topics and dates by a circular mail.

Participants in the UKE PhD programme will receive 1 credit point for every 10 CVcare Journal Clubs they visit.
